11) Employment Summary: Click Update.

12) Florida Employer Search: Enter the employer FEIN found on your W2 or the employer name Elevated, LLC, then click ‘Search’. Our company should be the first one on the list with an auto-populated Tallahassee address.

13) Most Recent Work Information: Please reach out to HR if you need your ‘Start Date’ with us. You will need your W2 or pay statements to enter your gross wages. Please visit www.workforcenow.adp.com to access this information.

14) Reason for Separation: Select “Layoff”.

15) Return to Work Information: Answer “No” to Question 1, otherwise it will ask for a ‘return to work’ date.

16) Provide Additional Employment: Click ‘Add’ and enter any additional employment information if you worked elsewhere in the last 18 months.

18) Work Search Requirements: Once your claim is submitted, the system will ask you to complete the work registration process through Employ Florida, although the requirements may be waived during this time.
